The Fallbrook Commons community is thoughtfully designed with three distinct neighborhoods within the building – Deering, Libbytown, and Munjoy.

The design of the community, with a focus on private rooms and private bathrooms, allows our staff to provide high-level clinical care in a comfortable environment, and maximizes the quality of life for people we serve. Each neighborhood features a residential style kitchen, comfortable dining and living areas, and a secure outdoor courtyard.

Nursing Care

Our short-term care programs provide services to patients who have been discharged from a hospital and need rehabilitation to return home. Long-term care are for residents that aren’t able to return home safely.
